Our medical home model emphasizes high-quality, safe and effective care, placing patients at the center of the health care delivery team. Your care team will consist of your primary care physician, nurse practitioners, registered nurses, licensed practical nurses, medical assistants, care management team, and front desk administrative staff.
Our physicians and nurse practitioners are board-certified, and will work collaboratively to assist you with coordinating your healthcare needs. They provide routine physical exams, wellness screenings, preventative medicine, and the diagnosis and management of complex medical conditions, referring you for specialty care when needed. They are available to answer questions, and provide patient education, so that you feel comfortable making informed decisions about your healthcare choices.
Controlled Medications
Please be advised that because controlled medications are dangerous, can be addictive, and carry legal risks, Martha’s Vineyard Hospital Primary Care has the following policy regarding controlled substances: It is our goal to build a relationship with each patient and carefully assess the needs and issues of each patient before prescribing controlled medications. Our Practice does not routinely provide ongoing prescriptions for narcotic or benzodiazepine medications. (Your pharmacist will be able to tell you if your medication is a controlled substance.) Please discuss your medical needs with your assigned provider. If you need an urgent refill of your medications, please contact your prior provider.
All Refills
Patients have several options to refill prescriptions. Please plan, so that you do not run out of medication.
- Order online using Patient Gateway
- Ask your pharmacy to send an electronic refill request to us
Prescription requests received between 9am and 4pm, Monday through Thursday will be filled within 24 hours. Prescriptions received after these hours will be filled within 2 business days. If an authorization is needed from your insurance company for the medication, it may take longer.
- Annual exams should always be scheduled with your primary care physician or nurse practitioner. Please call us or schedule an appointment online using Patient Gateway.
- We reserve appointments each day for existing patients who have an unexpected illness or injury. Please call to request an urgent appointment. Our practice reserves time for same day routine and urgent care appointments based on patient preference and practice triage. Urgent needs are given priority. Please do not use Patient Gateway for an urgent care request.
- We make every effort to ensure prompt service for all our patients. To help us achieve this goal, please plan to arrive 15 minutes before each appointment.
- If you need to cancel an appointment, please notify us at least 48 hours in advance. This makes the appointment time available to another patient who may need it.
- If you missed your appointment, the staff will call you to reschedule. If you miss 3 appointments in one year without timely cancellation, we will contact you to discuss you continuing in our practice.
